Sourcewise, is an independent non-profit organization serving Santa Clara County since 1973. The mission of Sourcewise is to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options. Community Resource Specialists at Sourcewise connect individuals with experts in the areas of health insurance counseling, Meals on Wheels, senior employment, care management, and other services available throughout Santa Clara County.
